9. H&S REPORTING AND
ANALYSING ACCIDENTS AND
INCIDENTS
Monthly reporting on H&S topics and performance will be
complete so that lagging and leading indicators for
improvement can be identified. Accident and Incident
reporting and investigations are essential to identifying root
causes and preventing recurrence whilst improving
performance across the business
9.1 Accident and incident investigations will include
identifying root causes and relating back the corresponding
section of the business management system.
9.2 All major investigations will be conducted by a multi-level
team that will include input from the senior leadership team
where required.
9.3 All physical and occupational health, safety and
environmental incidents and learning events/near misses are
openly reported, investigated and analysed without fear of
recrimination or blame.
9.4 Good practices, lessons learned and preventative actions
will be shared across the company. Project personnel and
leaders will take appropriate action and use the information
accordingly upon receipt.
